- The company name and address for where the waste was generated
- Your generator status, and EPA ID number if you have one.

- The process that generated your waste
- For example: “Expired/unused product”, “Solvent used for parts cleaning”
- The name of each waste stream, how much you have, and what its stored in:
- 5,000 gallons of Wastewater in a storage tank
- 1 x 275 gal totes of Cutting Fluid
- 5 x Cubic yard boxes/bags of Solvent Contaminated Rags
- 10 x 55 gal. Drums of Sodium Hydroxide Dip Solution
- Chemical components by % for each waste stream:
- For example: Sodium Hydroxide 15-25%; Water 75-85%; Dirt and Scale 5-10%, Lead 7 ppm
- Any applicable SDS’s or Analyses
- We can pull samples and run any needed analysis
